My Health Record Notice This organisation has been granted an extension of time to upload pathology reports to My Health Record. During this extension period, diagnostic imaging reports and/or pathology reports provided by this organisation will not be uploaded to My Health Record. Your results will continue to be sent to your requesting healthcare provider.
All request forms sent electronically must comply with the Privacy Act 1988, Health Records Act (Victoria) 2001 and the Information Privacy Act (Victoria) .
